What To Expect:

Consultation: 
My initial consultation is a two-hour diagnostic interview where together we explore your chief complaint, current symptoms, current medications, past medications, medical history, psychiatric history, and your goals for treatment.  Together we will devise a treatment plan that works for you.  I will provide thorough education in regards to diagnoses, interventions available to you, potential treatment plans, and referrals if need be. You will decide, based on the information provided, if what I am offering works best for you.    

Medication Management with Therapy:
If medication is something that is appropriate for you, and it is something you wish to explore or continue utilizing and you also find benefits in therapy, a medication management with therapy appointment is an option for you.  These visits are 45 minutes in length and combine both modalities.

Individual Therapy:
In the case where medication may not be appropriate for your treatment plan, or it is something you do not wish to try, individual therapy sessions are available.  These visits are 45 minutes in length, and we will approach them with whatever modality is best for your diagnosis and your specific needs.  

Medication Management:
Medication management appointments are 30 minutes in length and are designed to evaluate your current psychiatric medications and make adjustments if needed.  We will thoroughly explore the benefits, side effects, resulted lab values, and any questions or concerns you might have during your appointment time.

Frequently Asked Questions (FAQ):

How do you know if we will be a good fit? 
My philosophy is simple – if you do the work, you will succeed.  The path may not always be linear, and the journey may not always be pleasant, but on the other side of your work, lies progress and growth.  We will go at your pace, but we will always try to be moving forward.  I believe that everyone has the ability to realize their goals and live a happy and healthy life and it is my job to help you get there.  

Do you accept any insurance? 
I am an out-of-network provider and will provide any documentation necessary for you to be reimbursed using your out-of-network benefits if you have them.  I also do not participate in single case agreements with insurance companies so it is best for you to call your insurance company prior to scheduling an appointment so you can properly assess what the end cost will be to you.  Please see below as a guide to begin the conversation with your insurance company:

“Hello, I am hoping to schedule an intake appointment with a psychiatric nurse practitioner and will likely be receiving ongoing treatment.  She does not accept any insurance or participate in single case agreements so I am calling to find out what my out-of-network benefits are.  Her name is Candice Miller and her NPI # is 1811586464.  She may use some of the following CPT codes: 90792, 90862, 90836, 90834.  Based on this information, can you tell me what percentage or dollar amount I can expect to be reimbursed?”

You may also want to ask:

  1. Is there a maximum amount covered for office visits in a calendar year?
  2. Do I have a deductible and if so, how much is that?
  3. Do I need pre-authorization? If yes, can you grant me pre-authorization with the information provided?
  4. Is there a specific claim form I must use for reimbursement and if so, where can I find that form?
  5. How much time do you allow to submit claims after each visit?

Rates:
Consultation (2 hour) – $350
Medication Management (30 minutes) – $150
Medication Management & Therapy (45 minutes) -$180
Individual Psychotherapy (45 minutes) – $150

What form of payment do you accept?
Check or Credit Card

When is payment due?
Payment is expected at the time services are provided and are payable by check or credit card. If you do not have out-of-network coverage, and my rates are cost prohibitive for you to receive treatment, please ask about my sliding scale.

What is your cancellation policy?
I kindly request that cancellations be made no later than 24 hours prior to your appointment time.  If you do not cancel your appointment prior to 24 hours, or do not show up for your scheduled appointment, you will be charged the full fee of your visit.  I request this graciously as it allows me to offer services to others in need should you be unable to attend.
